At FORM, we are committed to your peak performance; if your gear doesn't meet your expectations, you can return or exchange it within 30 days of delivery. All items must be in their original, unworn condition with tags attached to be eligible for a full refund.
Initiate Your Request
Visit our online returns portal and enter your order number and zip code to select the items you wish to return or exchange.
Print Your Label
Once your request is approved, a prepaid shipping label will be generated; simply download and print it to prepare your package.
Pack and Ship
Securely pack your gear in its original packaging if possible, attach the prepaid label, and drop it off at any authorized carrier location.
Receive Refund
Once our warehouse team inspects your return, we will process your refund or ship your exchange within 5-7 business days.
